1. This assignment assumes you have completed reading Schutt, ch. 8, "Survey Research", as well as Homework Set #4.
2. Currently, WSU Tri-Cities' Student Affairs Dept. has two separate student information online survey/ information request forms: a general "Information Request Form", and a separate "Freshmen Information Request Form". They would like to combine the two online survey forms into one, and would appreciate your feedback and assistance.
3. Go to and study the two online survey forms:

and

4. Answer the following questions:
a.) How can the forms be combined?
b.) What suggestions do you have for improving the online survey?
c.) How can wording and structuring of existing questions be improved?
d.) What additional questions might be useful?
5. Come to class on Tues., Feb. 13, prepared to discuss your suggestions with a representative of the Student Affairs Dept.
